Who pays for your Medical Care?
We accept payments from a number of medical insurance plans. A list of the major plans we honor is available here. If we are the primary care provider on your plan, we will accept your valid insurance card as payment for the services we provide for you. Of course, if a co-payment or a deductible applies, you will be required to pay this at the time of service. As a courtesy, we will file a claim on your behalf directly to your insurance company. Since insurance benefits (including co-payment and deductible amounts), billing addresses and eligibility change frequently, we rely on the information printed on your insurance card to file your claims properly. Please update this information at the time of check in when you pay your co-payment. Your new card will need to be scanned into our system.
Self-Pay
If you do not have medical insurance, you will be considered a Self-Pay patient. Self-Pay patients must pay for the medical services they receive from us at the time of the visit. We accept cash, personal check, VISA, Master Card, American Express and Discover.
Workman's Comp
Our practice does not accept Workman's Comp cases. If you are hurt on the job, you should contact your personnel department to obtain your company's authorized Workman's Comp physician.

We contract with most insurance companies. Please contact our office if your insurance company is not listed. We are not accepting any new Medicare patients at this time.
